Even weirder: I had a game once where I found my character's items (only the ones he had equipped) scattered on the floor of dlvl 3 or 4. Nobody in the game was a cheater (Private game -- a variants game from ages ago -- so I'm quite confident) and nobody'd been to that dlvl. My character had neither died nor dropped his items prior to finding them scattered about.

Haven't seen that. But I've seen Monsters do it. Usually in Hellfire MP, I've seen monsters drop Item A, and soon after a monster drops Item B. Item A morphs into Item B; when either is highlighted, both items highlight. In all cases, one of the items disappears -- still rather weird. My suspicion: Desync between computers regarding which monster should drop what.

Hehe, that's really bad desync. In those sorts of games, players usually get an hourglass every few seconds.

Hmm. It can recur but I wouldn't know how to reproduce it deliberately.
